Informed Consent

Purpose
You are invited to participate in a research project exploring the impact that the COVID-19 pandemic has had on your personal life. There have been many recent changes that have affected people in different ways, so, even if the illness itself did not affect you or someone you know, we are seeking your response. In conducting this study, we hope to determine how the responses to this pandemic (including cancellations, school closures, social distancing, etc) impacted those in the United States. The data collected will be used for publication in book form and peer-reviewed academic journals.
Voluntary Participation
Your decision to participate in this study is completely voluntary. If you decide to participate, you can expect the following: you will complete an anonymous online survey which will ask you for demographic information followed by questions addressing the effects of the current COVID-19 situation on your life. You have the option of writing your story (a few sentences or multiple pages), completing brief survey questions, or doing both. Your total time to participate in the survey portion will be approximately 5 minutes. If you write your story, there is no minimum or maximum time.
Anticipated Risk
Participation in this research is unlikely to result in risk to participants. In the event that completing this questionnaire causes emotional or psychological discomfort, please contact the Substance Abuse and Mental Health Services Administration (SAMHSA) national hotline at 1-800-662-4357.
Anonymity and Confidentiality
Please be aware that any information that is obtained in conjunction with this study and that can be identified with you will remain confidential and will not be disclosed at any time. There will be no compensation for participating in this study. Your decision whether or not to participate will not prejudice your future relations with the investigators or their affiliated institution, Stockton University.

You may skip questions and/or withdraw from this study at any time without prejudice should you choose to discontinue participation.
